Horizon Blue Cross Blue Shield of New Jersey plans to help members monitor diabetes with cell phones, under an agreement with Livongo, provider of a chronic condition management platform.

A new Medicare Advantage plan will rely heavily on analytics to focus efforts on treating chronic conditions.

The goal is to avoid traditional time-consuming tasks of communicating via fax, mail, email, courier and web portals, while reducing costs and speeding processes.
