Blue Cross and Blue Shield of Florida has notified almost 3,500 members that in early April their explanation of benefits or monthly health statement were inadvertently mailed to incorrect prior addresses because of a system change.

The plan, which says it has corrected the problem, is not calling the incident a breach, but rather a "mailing error." It has, however, sent notification letters to affected members and notified the Department of Health and Human Services' Office for Civil Rights, which enforces the health care breach notification rule.
